This streamlined approach to care will increase access to effective depression treatment for patients in Central Wyoming, decrease depression, and improve the overall well-being of the individuals and families served by CHCCW.
With the CHCCW IMPACT Team you receive the best care if you participate actively with your primary care provider. Your primary health care provider (PCP) will oversee all aspects of your care at CHCCW. Your provider will work closely with the other members of the care team to make sure you receive the best care possible.
Care managers work closely with you and your provider to implement your treatment plan, and to monitor your progress. The psychiatric nurse practitioner is an expert consultant to the primary care provider and the care manager. The psychiatric consultant is available to advise your care team about diagnostic questions or treatment options, to ensure you are receiving the best care possible.
CHCCW’s IMPACT and Behavioral Health services offer comprehensive depression and suicide prevention services. CHCCW offers free depression screenings to diagnose and treat the symptoms of depression and anxiety that can often lead to suicidal thoughts.
Our care managers will assess symptoms and discuss best available treatment options for you. Care managers are trained to provide professional and quality treatment in a confidential setting.

With support from the University of Washington’s AIMS Center, the John A. Hartford Foundation, Margaret A. Cargill Foundation and the Kinskey Family Foundation, CHCCW has served over 700 patients through the IMPACT Collaborative Care Model specializing in a multifaceted psychiatric treatment approach.
With the IMPACT Model, patients receive the collaboration from primary care providers, psychiatric consultants and a care management team to help manage depression.
